Add attendee forms

You can use forms to gather additional information for individual attendees or collect other information that needs to be stored outside of a specific signup, like a medical release form. Forms are attendee-specific and available after a registration is submitted.

There are four types of forms you can add to the signup:

  • Attendee information form: This form is built into the registration process in Church Center. It allows you to require specific questions to be completed before the registration is confirmed for the attendees.

  • People form: Create a form in Planning Center People that will keep profile information up-to-date and can be viewed for the attendees.

  • PDF form: Attach PDF forms that must be completed and returned by the attendee. A PDF form lets you control the format or require a signature.

  • External form: An external form is a form created by another program that can be attached by a link.

Create a new form

To get started with forms, head to Forms on the Configuration tab of your signup. The attendee information form is built into all detailed signups, but you can add other forms as well.

To add another type of form to your signup, select New form and choose the type you want from the dropdown.

You may need more than just personal information on attendees or the registration contact, and you can gather that information by asking questions. Answers will be saved to the attendee's registration form.

Important

Questions in the attendee information form will not update profile information in the database or be accessible from the Forms tab on profiles in People. To update profile information, including custom fields, use a People form with profile fields.

Build your form by adding questions specific to selections and keep them organized with section headings. Each attendee will only see questions marked for their specific selection or all selections.

Warning

The attendee information form cannot be deleted, and you cannot create more than one. Edits must be made to each question individually.

Add a question

From the attendee information form, select Add a question to create a question for attendees to answer.

Set up the question as you want it to show when someone is registering.

  • Choose the type of answer you want to receive.

  • Enter the question in the way you want it to be shown on the event page, and then enter options, if applicable.

  • Select how you want this question to be shown on Church Center.

  • Choose to ask this question for only certain selection types.

Select Save to add the question to your event and view a list of all other questions.

Add a section heading

Use a section to split up your questions. Select Add section heading, and then enter the name and description.

Drag the section above the questions it applies to.

Select a header to edit or delete it.

Mark PDF and external forms as complete

Unlike the attendee information form and People forms that are automatically marked as complete in a registration, downloadable PDF forms and external forms need to be manually marked when they're turned in.

From a person's registration details, check the box next to the form to mark it as complete.

Send a form due reminder email to an individual or all attendees who haven't completed or turned in their form from the Actions dropdown on the Registrations tab. The email will have a link to the form they need to complete.
